Clinical Features of Hepatocellular Carcinoma in Young Patients

Abstract

|
Background/Aim: The clinical features in young cancer patients are often different from those of older patients. The purpose of this study was to evaluate the differences in clinical characteristics of hepatocellular carcinoma between
younger and older age groups.
Materials and Methods: We retrospectively studied the clinical features of 371 patients with pathologically-proven hepatocellular carcinoma who underwent curative hepatic resection between 1991 and 1995. The patients were divided into two groups: younger age group(n=131); younger than 50-years-old, Older age group (n=240); 50 or older.
Results: The younger age group showed a higher incidence of positive hepatitis B surface antigen (87% vs 65.8%, p<0.0001) and a lower incidence of positive hepatitis C virus antibody (4.2% versus 26%, p<0.001). Indocyanine green retention rate at 15 minutes was lower in younger patients than lower patients (8.6% vs 11.8%, p<0.05). The size of tumors in the younger age group were larger than that of the older age group in diameter(5.3§¯ vs 4.4§¯, p<0.02), and serum levels of alpha-feto-protein were higher in the younger age group(p<0.001). Major hepatic resections were more frequently performed in the younger age group(15.3% vs 8.8%, p<0.05), but postoperative complications developed more frequently in the older age group (16.0% vs 26.7%, p <0.05). The overall survival did not show any significant difference between groups(5-year survival rate; younger 56.1% vs older 61.0%).
Conclusion: The prognosis of hepatocellular carcinoma in younger patients is comparable to that of lower patients, although younger patients haute more advanced diseases. Routine screening is recommended to detect the tumor at an earlier stage in younger hepatitis B positive patients.
